Use the form below to request Libre 2 or Dexcom One sensors. You can also use the form to change the quantity of your needles/test strips/insulin or sensors (within reason). This service is available for patients registered with:

You will need to buy some alcohol wipes from the pharmacy as these do not come in the box.

If you have not used Dexcom One sensors before you will need to download the Dexcom One App. You can check if your phone is compatible by checking on this page. The App includes instructions on how to apply your new sensor which lasts 10 days.

You need to buy some alcohol wipes from the pharmacy as these do not come in the box. You will also need a Dexcom One Transmitter. These last 3 months and your pharmacy should order one of these free devices for you.
