Practice Boundary


If you live inside the Milford Boundary (Purple) then you should register with Milford Surgery and will be routinely seen there.

If you live inside the Wistaria Boundary (Yellow) then you should register at Wistaria Surgery and will be routinely seen there.

If you live inside one of the orange areas then you can only be registered with the surgery if you have moved from inside the purple or yellow areas

Registration

If you would like to register with the Practice please call into reception and speak to our reception staff. We will provide you with a simple registration form and a questionnaire that you will need to complete. Both forms can also be downloaded using the link below. 

Please bring photographic identity and proof of address so we can ensure your personal information is correct and we can locate your previous Surgery to access your past medical records.

It can take some time for your records to reach us from your previous doctor, and the information you give us in the questionnaire is helpful in the interim.

For families with children, we emphasise the particular importance of recording the immunisation and vaccination history.

All new patients are offered a health check; this appointment enables us to take some details about your medical history and some simple measurements e.g. blood pressure.

Allocated a Named ‘Accountable GP’

Once registered with Wistaria & Milford Surgery, you will be allocated a named ‘Allocated GP’.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients. You do not need to see your named GP when you book an appointment at the surgery.

You can see a variety of health professionals in addition to a GP, such as:

  • Physician Associate 
  • Practice Nurse or Health Care Assistant
  • Musculoskeletal (MSK) First Contact Physiotherapist
  • Paramedic
  • Health and Well-Being Team
  • Pharmacist or Pharmacy Technician
