Annual Review Processes

Asthma

First invitations will be by text message. Invitations will be sent by post to those without a mobile number on record. Due to COVID these reviews are currently being undertaken over the phone. If needed a face to face appointment may then be arranged.

COPD

First invitations will be by text message. Invitations will be sent by post to those without a mobile number on record. Due to COVID these reviews are currently being undertaken over the phone. If needed a face to face appointment may then be arranged.

Diabetes

First invitations will be by text message. Invitations will be sent by post to those without a mobile number on record.

You will need to have had a recent blood test before you book your appointment. If you have a home blood pressure monitor at home it would be great if you could bring 5 days of home readings with you. For instructions on how to do this please visit this page of ours.

Heart Failure

First invitations will be by text message. Invitations will be sent by post to those without a mobile number on record.

You will be asked to book an appointment with a Health Care Assistant who will review your height, weight and blood pressure. They will also discuss issues like breathlessness and leg swelling. If you have a home blood pressure monitor at home it would be great if you could bring 5 days of home readings with you. For instructions on how to do this please visit this page of ours.

Hypertension (High Blood Pressure)

Those with high blood pressure should have a blood pressure reading annually at a minimum. The best way to check your blood pressure is to do 5 days of home readings annually. You can find details on how to do this at this page of ours.

Many blood pressure medications require an annual blood test. You will be contacted when this annual blood test is due.

Ischaemic Heart Disease

In the past we invited all those with Ischaemic Heart Disease for a blood test and an appointment with a nurse or health care assistant. We now will be in contact to arrange a blood test annually if indicated based on your specific medication. If you are taking a Statin then having your cholesterol checked every year is no longer recommended.

We would recommend submitting home blood pressure readings each year. You can find details on how to do this on this page. We will not be inviting all for a face to face appointment but you can book a blood pressure appointment with a health care assistant if you would like to.

Mental Health

An annual appointment with a Health Care Assistant following a blood test is recommended. This review is focused on reducing your risk of heart disease, stroke and elevated sugar levels. First invitations will be by text message. Invitations will be sent by post to those without a mobile number on record.
